American Kidney Fund: 2018 SJ Kidney Action Day

American Kidney Fund is interested in working with parishes to help promote and advertise our event and to encourage parishioners to volunteer or attend free health screenings. Our signature outreach event is held in cities where the rates of kidney disease are higher than the national average. Our goal is to encourage people to embrace good health through education, screenings and resources available. No RSVP is needed if parishioners would like to attend. For volunteers, please see attached a Volunteer Packet for review and a waiver form which will need to complete and email back to kfarzin@kidneyfund.org by March 20, 2018. It is recommended taking a picture or scanning the form and adding it as an attachment or completing the form as a PDF file
